Facility had not notified her daughter(who was out of town) that she had been transferred in. I called her to let her know. She then called the DON to complain. Daughter is an RN who had worked there a few years ago. DON told her patient …”

“My mother, Gloria Q. Bernal, was a patient at CCNRC for about 10 months. She would like to say, "Thank you for taking such great care of me. The staff made me feel like my extended family. I love everyone at CCNRC. I would like to personally thank the CNAs: Amanda, Britney, Isabel, Samari, Ayana, Melissa, Art, Clifford, Allen, Charisse, Will, Breanna, Brenda, Paige, Lupita, Yvette, Jennifer, and A…”

“Conditions were absolutely horrible and unsanitary. The place smelled like urine. The staff was very rude. They were yelling and screaming at other patients and and Mr. Pope and it's just unsetti, I don't know I wanna call adult protective services that's exactly what I wanna do, so I'm not happy with the condition we had to take him out of there immediately due to everything. Thank you Take”
