Banned from Medicare? The exclusion list knows.

The HHS Office of Inspector General maintains the LEIE — a list of 83,000+ people and businesses excluded from Medicare and Medicaid for fraud, patient abuse, license revocation, and similar findings. Hiring or being run by an excluded party is itself a violation (facilities have paid seven-figure settlements for it). Senior Home Review cross-checks the full list against every facility and registered owner monthly.

Search the facility

Find it in the facility search. If a facility's own provider number, or an entity matching its registered name in the same state, appears on the exclusion list, an OIG exclusion list line shows in its Regulatory standing section — with the exclusion date and the reason category decoded (patient abuse, health-care fraud, license revocation, and so on).

Understand what a match means — and doesn't

We flag publicly only the strongest matches: the facility's own federal provider number, or an exact registered-name match in the same state. A name match is strong evidence, not a legal determination — always verify at the source before acting on it. Name-only matches to individual officers are deliberately NOT shown (names alone can't prove identity); we review those internally.
