Find it by name, state, or ZIP in the facility search. Chains often operate under building-level LLCs — the page shows both the brand name and the legal entity.

Open “Registered owners & officers”

In the Facility background section: every registered person and entity, with role (direct or indirect owner, officer, managing employee), ownership percentage, and the year their stake began. The 2023 transparency rule's “additional disclosable parties” — management companies and service providers — are listed and labeled in plain English.

Read the ownership context

The same section shows the operator of record, the parent company or chain, and the for-profit / non-profit / government classification — plus how recently ownership changed, which research links to changes in care quality.

Why look it up here

The underlying filings are public, but CMS publishes them as a 249,000-row spreadsheet keyed by certification number. Here they're matched to the facility page you're already reading, with roles decoded and the trust-and-LLC web laid out — alongside the inspection record the same owners are responsible for.
