Search the facility by name, state, or ZIP

Use the facility search (or the map if you only know the area). Every certified nursing home in the country has a page.

Read the Inspection record section

Each facility page lists every citation from the last three years, grouped by inspection date: the tag (like F-812 for kitchen sanitation), a plain-English description, whether the visit was complaint-triggered, and the federal scope/severity grade — from “no harm” up to immediate jeopardy, the most serious grade CMS issues.

Check penalties and the food-safety record

The same page shows every federal fine with its date and amount, Medicare payment denials, and a dedicated Kitchen & food safety verdict — 20,020 food-safety citations are on record nationally, and most listing sites never show them.

Compare against the state

Every state hub (e.g. Ohio) summarizes statewide citations, immediate-jeopardy counts, and average ratings, so you can tell whether a facility's record is typical or an outlier.
