“Very poor communication within the facility. Family member was high risk for falls, and she fell twice, and neglect was substantiated both times. I asked that a chair alarm be used and was told the alarm would disturb the other residents. The second fall required a visit to the ER and two days later the facility decided to use a chair alarm. A little too late. I would advise looking at other facil…”
