“Do not send your loved one to this facility. My grandmother was neglected at Oak Leaf Manor North. After three weeks at Oak Leaf (FROM 12/19/25-1/13/26), I was never told that the pull cord doesnt work in the residents room and the resident ended up having countless falls because they were unable to alert staff when needing assistance.
