“Great rehab facility!! Staff and workers were all very friendly, helpful and did great job on my husband’s therapy. Really nice size private room with a window. Facility is kept very clean. The food was very good also. Would highly recommend.”

★★★★★Julie Whitney · 2026-03-16 · via Google

“Scott Lake Health & Rehab is one of the best facilities in Polk County. My mother has been here twice for physical rehab and now is part of their long term care unit. It's an extremely clean facility and the staff goes out of their way to care for their patients and residents. Special shout out to the nurses, who truly care, including Charity, Kelly, Trisha, Bella and Tiffany (nurse manager). And,…”

★★★★★Mary Fra · 2026-06-30 · via Google

“My mother has stayed here twice for rehab in the last 9 months. For the most part, the facility and staff are very good. Staffing can be overwhelmed with their high number of patients. That said, there are some staff members that go above and beyond. Alvin is excellent and a great nursing administrator. Our thanks to all the staff for their care.”
